Gilmerton Neurological Care Centre is a specialist service in South Edinburgh, providing personalised care for individuals with acquired brain injuries and progressive neurological conditions. We work in close partnership with NHS services and local authorities to support appropriate, timely transitions from hospital to residential care.

Specialist Neurological Care in a Residential Setting

Gilmerton offers a safe, structured environment for individuals requiring ongoing clinical care, slow stream rehabilitation, functional therapy, and palliative support. We are the only Headway Accredited Provider in Scotland, with a multidisciplinary team experienced in delivering complex care in line with evidence-based practice. Our service is well-suited to those who need ongoing assessment, therapeutic input or long-term support in a residential setting.

Neuro Rehabilitation

Gilmerton provides specialist slow stream rehabilitation for individuals with acquired brain injury and a range of neurological conditions, including Parkinson’s disease, Multiple Sclerosis, Motor Neurone Disease, stroke, and epilepsy. Our service is designed for those who may continue to make progress after discharge from hospital, but who require extended support to do so safely and effectively.

Therapy goals are set collaboratively with the individual and their family, and delivered by a team including physiotherapists, occupational therapists, and NHS neuro-rehabilitation partners. Programmes may include mobility work, communication and cognitive therapies, activities of daily living, and confidence-building interventions, all tailored to the individual’s potential for improvement.

Palliative Care

We offer responsive, person-centred palliative care for individuals living with advanced neurological conditions and those requiring end-of-life support. Gilmerton participates in the 7 Steps to Anticipatory Care Planning programme in partnership with Edinburgh Health and Social Care and receives support from Marie Curie professionals, ensuring high-quality and coordinated palliative delivery.

Our team provides 24-hour nursing care in a calm, homely setting, focusing on symptom management, emotional support and maintaining dignity. Advance care planning is conducted with the individual and their family wherever possible, and we work closely with GPs, palliative consultants, and local services to ensure the resident’s needs and preferences are fully respected.

Nursing Care

Gilmerton provides 24/7 nursing care delivered by experienced registered nurses and a trained care team, led by a Clinical Nurse Lead Coordinator. Our nursing team is skilled in managing complex care plans for individuals with progressive neurological conditions and long-term disability, including those requiring PEG feeding, epilepsy monitoring, diabetes management, and wound care.

Care plans are developed with clinical input and reviewed regularly to reflect changes in condition. We liaise closely with community healthcare teams, GPs and consultants to support continuity of care and provide timely reporting and involvement in reviews as required. The environment is fully accessible, and nursing care is delivered with dignity, safety, and person-centred practice at the core.

Functional Therapy

Functional therapy at Gilmerton focuses on improving everyday ability, participation and independence for individuals with neurological deficits. The therapy team provides assessment and intervention in areas such as executive functioning, problem-solving, personal care routines, and engagement with meaningful activities.

Therapeutic work is goal-driven and embedded in daily life, promoting practical gains and supporting cognitive and emotional wellbeing. We work closely with psychological services, speech and language therapists and external occupational health services where needed. Our approach supports residents to re-engage with daily routines and, where appropriate, prepare for supported return to work, volunteering, or community activity.

Gilmerton Neuro Care Centre offers a flexible and clinically responsive approach to both long-term placements and shorter-term step-down care. Our team is experienced in supporting adults with complex nursing needs, neurological conditions, dementia and enduring mental health diagnoses.

We work closely with referring professionals, families and the individual to ensure that Gilmerton is the right fit and that the transition into our care is smooth and well-managed. Following your referral, our Clinical Team will carry out a prompt and thorough assessment, either in person or remotely, depending on the prospective resident’s location and needs. This collaborative process ensures continuity of care and supports a safe, person-centred admission pathway.

We support adults with acquired brain injuries and progressive neurological conditions who require slow stream rehabilitation, long-term nursing care, functional therapy, or palliative support. Referrals are reviewed by our clinical team based on current care needs, rehabilitation potential, risk factors, and whether our environment is appropriate for the individual’s support requirements. We accept both NHS-funded and privately funded placements.
